Podcast Participation Agreement

Please review and complete this form to provide your consent for participation in our podcast. By submitting, you agree to the terms for using your voice and image as part of the podcast content.

1. Consent to Recording and Use

By signing this release, you consent to:

• Being recorded in audio and/or video format during your participation.
• The use of your voice, image, name, and likeness in connection with the podcast and any promotional materials.

2. Grant of Rights

You grant the podcast producers (hereafter referred to as "Producers") the following rights:

• The irrevocable, perpetual, and worldwide right to use, reproduce, distribute, modify, publicly display, and publish the recording or any portion thereof in any medium or format, now known or later developed.
• The right to edit or modify the recording for production purposes, provided it does not misrepresent your contributions.

3. Compensation

You acknowledge and agree that:

• You are not entitled to compensation for participating in the podcast.
• Any payment or reimbursement will be subject to a separate written agreement, if applicable.

4. Waiver of Rights

You waive any rights to inspect or approve the final product or the manner in which your voice, image, or likeness is used. However, the Producers will make a reasonable effort to ensure that your contributions are not misrepresented and are presented in a manner that best reflects your intended message and character.

5. Release of Claims

You release the Producers, their affiliates, and any associated parties from all claims, demands, and liabilities related to the use of your contributions, including any claims for defamation, invasion of privacy, or infringement of rights of publicity.

6. Confidentiality

You agree not to disclose any proprietary or confidential information shared during the recording unless granted permission by the Producers.

7. Legal Age and Authority

You affirm that:

• You are at least 18 years old or have obtained the consent of a parent/guardian if under 18.
• You have the authority to grant the rights outlined in this form.

8. Entire Agreement

This release constitutes the entire agreement between you and the Producers regarding your participation in the podcast. It supersedes and replaces any prior or contemporaneous discussions, agreements, or understandings, whether written or verbal, related to the subject matter herein.
